A: Almost always a spring that is too strong for it, or a gear train that is not shimmed straight. Fixing the piston without fixing the cause just breaks the next one.

Q: Do I need to grease it?

A: Light grease on the piston O-ring and cylinder wall, nothing on the piston teeth beyond what the gears carry. Over-greasing causes hydraulic lock and slows the cycle.
